Pit Bull Release Form

For those shelters that are required by law to have a pit bull release form, we now have this available in Shelter Buddy. When an animal is entered that has a primary or secondary breed of "pit bull", a new "Pit Bull release form" link will show in the animal menu. From here you can print the form and soon you will also be able to have the form signed digitally and email it. 

To create your form:

1. Go into Administration
2. Select Templated Document Administration
3. Select Animal Forms (Printed)
4. Click +Create
5. The Templated Document page will load. 
6. Location: If each of your Physical Locations is to have a different template, you can select from this drop-down and set each one up in turn. You can also set a default if there is more than one version for the same Physical Location. If the form is the same for all locations, leave this set at all. 
7. Description:  This free text field is how your users will identify which form is relevant to them, in their Physical Location. 
8. Type: Currently, there is only one option so this drop-down is not active. 
9. Category: Again, there is only one option at this time so just leave this field as-is.
10. Body: Click into the canvas below this drop-down to activate the place-holders.  You can set up the body canvas using basic tools such as tables, formatting, colour, etc. The placeholders allow you to auto-fill text for individuals. Auto-fill options include: 
    • The date the letter is generated
    • Animal information (name, breed(s), license/microchip number, location, gender, colours, age)
    • Person information (salutations, phone numbers, address)
    • Site information (organisation name, abbreviation, phone number)
11. Once you have completed your form, click SAVE to return to the Templated Document page. 

The form will now be available as a link in the Animal Menu on the right-hand side of any animal where the primary or secondary breed is "pit bull".
