Euthanasia Request Procedure

This is how to bring an animal into the system that is being surrendered by the animal’s owner or an agent of the owner to be euthanized.

  1. Check to see if the animal has ID or a microchip. Be sure to search the system to see if the animal already exists.  If the animal is found in the system, note the Animal ID number.  They may or may not be linked to the person surrendering them.
  2. Search for surrendering person.
    • If found, skip to step #3.
    • If not found, click on the Create a New Entry link or click on the New Record link in the orange menu to the left of the screen (if available).
    • Fill in the relevant blanks on the Add New Person Mandatory fields: first name, last name, residential address, mailing address if different than residential,   telephone number, and gender.  Depending on your organization you may be required to confirm identification and include date of birth.
    • Click Update Details.
  3. On the Edit Person Details page, click on the Scroll To Bottom link in the upper left hand section of the screen and click on the Euthanasia Request link in the Personal Categories
  4. On the Edit Euthanasia Details page click on the Add New Euthanasia Animal *
    • *If the animal is already in the system on the Edit Euthanasia Details page click on the Euthanize an Existing Animal
    • *On the Animal Search page enter the Animal ID in the Go To Animal ID Click View, and then click Select to confirm the displayed summary is the correct animal. 
  5. Complete the following(All Yellow fields are mandatory and may or may not be listed below):

Identification Details

  1. Source: Normally, the default source Euthanasia Request will be appropriate, however if the owner or agent has agreed that the animal can be rehomed if able to be saved, click the down arrow on the Source drop down to select Euthanasia Request with Consent to Rehome
    • For some sites, the source Pending Medical Evaluation may be available also. 
  2. Status: click on the down arrow and select the most appropriate status. The most common are Awaiting Euthanasia or Euthanasia / Cremation Request.
    • If the animal has any Identification on it should be filled in at this time. The ID Checked field should always be populated to confirm that there is no ID on the animal if all fields are blank.
    • There is an area for the confirmation of Microchip numbers as to their existence in the animal or not under Identification Confirmation details. This is the light blue area just under the Identification area on the system and is optional.

Animals Details

  1. Animal Name: enter the animal’s name if they have one (might be a sick unnamed baby animal).
  2. Age: enter the animal’s age by using the applicable weeks, months, years fields.  Identify if this is an approximate age. If the DOB is known, click on the Calendar Icon to select the date (remember to select year then month then day if the animal was born in a previous year) and the age will auto-calculate when you save the page.
  3. Class: The selection from this list will determine the Type.   DOMESTIC will default and should be used for cats, dogs, puppies, kittens and birds.  For small pets, such as mice, change this to Pocket Pet.  For unusual pets, such as snakes, change this to Exotic.  For farm animals, such as goats, change this to Livestock.
  4. Type: The Type list will in turn drive the selections available in the Breed / Species list. Click on the down arrow to select the type of animal this is.  EX: dog, cat,  etc. 
    • If cat or kitten is selected the Feral field will become mandatory.
  5. Breed/Species: Type in the first one to four letters of the animal’s primary breed and the field will present options in a drop down. Select as appropriate.  If the animal is a mixed breed, select the breed the animal primarily looks like. Add a Secondary Breed / Species if applicable. 
  6. Gender: Click the down arrow to select the animal’s gender.
  7. Circumstance: click on the down arrow to select Euthanasia.
  8. Primary Colour: The Colour list is driven by what animal Type is selected. Click on the down arrow to select the animal’s primary colour.
  9. Secondary Colour: The Colour list is driven by what animal Type is selected. Click on the down arrow to select the animal’s secondary colour.
  10. Complete any other highlighted mandatory fields required in this section. 

Status Details

  1. Date In/Found: Today’s date will default. Click on the Calendar Icon to select the date if this is incorrect. For Euthanasia animals, this should always be the same as the Date In Shelter.
  2. Physical Location: The Physical Location the user is logged in as will default. Click on the down arrow to select a different one if this is incorrect.
  3. Shelter Location: Click on the down arrow to select the appropriate location the animal will be placed in within your shelter.
    • If known, click on the down arrow and select the kennel number of the kennel the animal will be placed in. If not known, this can be done after the animal has been placed.
  4. Date In Shelter: This is the date the animal was actually brought into the shelter.  Today’s date will default.  Click on the Calendar Icon to select the date if this is incorrect

Lost / Found Details

  1. Jurisdiction / Shire / Council / Warrant Area: This field is nearly always mandatory. Click the down arrow to select the appropriate jurisdiction.
  2. For some sites, a City / Suburb / Township may also be mandatory. Enter the first few letters of the city into the text box and click Find Suburb/City. Select the relevant city from the resulting list to auto populate the city, post code, state and jurisdiction.

General Details

  1. Euthanasia Reason: Click on the down arrow and select the primary reason for the animal being put down.
    • Secondary Euthanasia Reason: Click on the down arrow and select the secondary reason for the animal being put down, if applicable.
    • Other Euthanasia Reason: Enter an additional reason here or enter a more specific reason if you cannot locate it on the drop down list in the previous field. E.g. selecting Sick from the drop down menu then further adding ‘Unable to stand up’ or ‘Refusing to eat’.

  2. Send Sympathy Card: *Optional* Place a check mark in the box if a condolences card or note is to be sent to the owner, this is usually only applied to animals that have come into the shelter specifically for euthanasia.
  3. Owner present for euthanasia: *Optional* Place a check mark in the box if the owner of the animal was present during the euthanasia process.
  4. Click Update Details.
  5. Depending on local settings, the Product Added page will load.
    • If there are fees related to euthanasia add them now by clicking Find next to the Product / Service text box and selecting from the list, or typing part of the product name into the text box and allowing it to auto-select from there.
    • Enter the quantity and click Add Product. 
    • Click Continue to Receipt
  6. The Add Receipt page will display.
    • Payment Method: Click on the down arrow to select the method of payment. EX: credit card, check, cash, etc. If no fee is required, click on the down arrow to select Free of Charge.  This page is displayed so that donations can be entered if needed.
    • Complete applicable fields based on the payment method.
    • Ensure there is a designated allocation selected if receiving payment.
    • If applicable, have your client sign digitally now. 
    • Click Update + Print.
  7. Print a copy of the receipt if your client requires one, or it can be emailed to them if you have this facility enabled.

Don’t forget to:

  • Print out a Kennel Card if the animal will not be euthanized immediately.
  • Add any applicable notes to the General Animal Notes link in the Animal Menu on the right side of the Edit Animal Details Usually the category of Ownership Notes would be used for a euthanasia request animal.
