Now that you've signed up for Activ4Pets and your clients have an opportunity to take advantage of a great app that lets them share important information with their local vets, what information does it pull from your site exactly?
This is a list of the fields that will be transferred to the Activ4Pets site to be shared with veterinarians:
Person Details:
- Person First Name, Last Name
- Person DOB
- Person Gender
- Person Email
- Residential Address
- Mailing Address
- Home Phone
- Mobile Phone
Animal Details:
- Animal Name
- Primary and Secondary breeds
- Primary and secondary colours
- Microchip number
- Animal Gender & Spay/Neuter status
- Adoption date
- Animal DOB
- Adoption "Source" i.e. which Physical Location it was adopted from
Medical Details:
- Vet Treatments
- Vet Consultations
- Vet Notes
- Vet Documents
- Medications
- Weight
*note, indemnities do not export at this stage but I believe they're planning to do that later.
*note: the free text fields that appear directly on the Medical Details page (i.e. General Vet Notes, Previous Medical Details, Known Allergies or Medical Conditions, External Veterinary Notes and Feeding Requirements) are NOT shared with Activ4Pets
To verify the information that is going to Activ4Pets, you can use the View Medical History link in the Animal Menu to see the information that will be pulled via the Medical History Report.
NOTE: You can set specific categories of Vet Notes to be withheld from Activ4Pets via Admin > Edit Drop Down Lists > Note Categories.
